Terms and Conditions
I understand that it is a condition of Guardian accepting my backup care booking that I agree to the following:
- Should my child suffer any illness, accident or medical emergency whilst in Guardian’s care, Guardian shall be entitled to seek and provide such urgent medical, dental, hospital treatment or ambulance service including transportation by ambulance service or assistance from the person or body nominated hereunder as deemed necessary for my child. I agree to pay all costs associated with such treatment.
- I consent to first aid being administered by a staff member who is a holder of a current First Aid certificate or is a registered nurse. I understand and accept that a first aid qualified staff member can only administer medication to my child that has been authorised by at least one of the parents/guardian and a qualified and registered medical practitioner, details of which have been accurately recorded in the Medical Action Plan.
- I give my consent to the administration of paracetamol in the appropriate dosage when deemed necessary by the Guardian employees. I am aware that Guardian employees will inform me verbally and in writing. I will also make Guardian employees aware of any medical administration given at home prior to arrival in the centre/the educator’s arrival in my home.
- I consent to the application of SPF30+ sunscreen to all unprotected areas of skin on my child, as necessary. Where in-home care is provided, I will ensure that there is sunscreen available for application.
- I consent to my child being taken to a park or similar location within a 15-minute walking distance by the Guardian educator.
- I will not ask Guardian employees to care for my child without making a booking through this program.
- I have read, understood and consent to Guardian’s backup care policies
- I understand that Guardian educators providing in home backup care are not provided with epi-pens. Where my child has a Medical Action Plan that stipulates administration of an epi-pen in the event of an anaphylactic reaction, I will provide the educator with an epi-pen with a valid expiry date.
